I'm generally comfortable nursing in public without a cover. However, these can be great for a baby who's distracted by a lot of people or movement and won't settle down to nurse if you're out in a busy place.
They're also great if you need to wear a dress, which usually necessitates pulling one side down and is a lot less discreet than lifting a shirt. I know there are great nursing dresses out there but they can be awfully pricy and having a cover means you can wear whatever dress strikes your fancy without having to worry about how you'll feed your baby in public.
The best part is being able to see the baby while nursing - I had an infant who liked to pop off and re-latch about a million times per feeding, and not being able to see what I was doing would have made it impossible.
Long story short, I don't see these as necessary if your baby budget is already tight, but they can be nice to have for certain circumstances (or if you just personally like a little more privacy). Pretty, too!
